  • and, HOMEBOUND AND TRANSPORTATION CHALLENGED

 

WHAT TO EXPECT

Each delivery/shipment, the menu of grocery items provided changes.

Home Delivery is once per week / Shipped Delivery is once per month.

The choice of delivery or shipment and day is made by Feeding GA Families.​

Vegan and allergy friendly options are available.

REQUIRED ACCEPTED FORMS:​

  • ----- Child's Birth Certificate

  • ----- DCFS Guardianship Letter

  • ----- SNAP Benefit Summary: Showing Child(ren) in Household

AND

  • ----- Medication Label: Cancer/Lupus/MS

  • ----- ADA License or Hang Tag

  • ----- SSI/Disability Benefit Statement

  • ----- Veterans VA ID card

  • ----- Hotel/Extended Stay Contract: MUST BE IN 30337, 30213, 30268, 30291, 30349

AND

  • ----- Proof of Address: MUST LIVE IN 30337, 30213, 30268, 30291, 30349
